A number of countries have announced major national projects to put this type of information system in place. This article adopts a comparative health systems approach to examine the experience of two smaller developed countries – Taiwan and Estonia – that now have universal national medical ac- count systems in current service. The comparison focuses on the structural similarities and differences of both core characteristics of these two health systems and of their specific personal medical account arrangements.

A combination of constrained public revenues with rapid technological and clinical change has created a particularly demanding set of operational challenges. Tax-funded health systems face three ongoing struggles: 1) finding badly needed new public revenues despite inadequate GDP growth 2) channeling additional funds into new high-quality provider capacity 3) re-configuring the stasis-tied organizational structure and operations of existing public providers. This commentary reviews key elements of this new information-revolution-driven context, followed by a consideration of seven specific policy challenges that it creates and/or worsens for tax-funded European systems going forward.

The novel element in Swedish health care is that the doctors are available as a public welfare service but by private providers. As telemedicine becomes more popular, the tensions in regards to financing and regulation are already in evidence and look set to rise further. We outline steps that could better integrate telemedicine into the health care system.

While healthcare delivery has traditionally been a push market, multi-sided markets offer the potential for transitioning to a pull market for service delivery. However, there is a need to better understand the business model for multi-sided markets as a first step to using them in healthcare. This paper addressed that need and describes a multi-sided market evaluation framework. Our framework identifies patient, governance and service delivery as three levels of brokerage consideration for evaluating multi-sided markets in healthcare.
